EHG’s Process for Developing Integrated Solutions

EHG is the launch pad for innovation in technology-enabled services for the management and self-management of chronic disease among disadvantaged ethnic minority populations. EHG is seeking partners who share this mission and seek to collaborate on the development and evaluation of technology-enabled services targeted to these populations.
To improve chronic disease management, EHG’s approach to the design of technology-enabled services involves the following components:

USER-CENTERED DESIGN

Patient and caregiver feedback is incorporated from the very earliest stage of the design process. The design team seeks to understand the social, economic, and cultural characteristics of the communities in which patients live, work, and receive care.

INTEGRATED TECHNOLOGY-ENABLED SERVICES

Patient and caregiver feedback is incorporated from the very earliest stage of the design process. The design team seeks to understand the Technology- Enabled Systems are developed to link patients to their health providers and to other critical community partners such as behavioral health providers and community pharmacies. Our services seek to harness the growing power of health teams in primary care (e.g. physicians, nurses, social workers, nutritionists, and health educators), building solutions that easily integrate into their workflow for prescribing Mhealth apps, monitoring symptoms and behaviors, and engaging patients in self-management.social, economic, and cultural characteristics of the communities in which patients live, work, and receive care.

CULTURALLY-ATTUNED SERVICES

Technologies are tailored for specific ethnic minority populations — thus far, including Latinos, African Americans, and Chinese Americans. Services are attuned to their cultural beliefs and behaviors related to health and the health care system, and adapted for language preferences, and for levels of technical and health literacy. Our multicultural team partners with community groups to ensure that technology enabled services are culturally and linguistically appropriate.

EVIDENCE BASED PRACTICES AND CLINICAL OUTCOMES RESEARCH

Products are developed based on a synthesis of evidence based practices for the prevention and management of chronic diseases. They are then tested in rigorous trials for their clinical effectiveness and cost effectiveness.

DATA ANALYTICS

Advanced data analytics are the foundation for population health management and our products.  Data mining and machine learning help tailor our recommendations and feedback to patients and users, and help them to stay engaged in self-management, increasing adherence to care plans and medications. sure that technology enabled services are culturally and linguistically appropriate.
